It was awful.
She cried a lot during the meal, which meant my husband and I took turns taking her out for a walk around the parking lot of the restaurant so we could finish our food before taking her home and putting her to bed. It’s been six years since that dinner out, and we honestly haven’t done a lot of restaurant dining as a family since.

I think we were slightly traumatized by that experience. It felt a lot easier to order takeout, go to Chick-fil-A, or simply cook at home. When my husband and I decided we really needed a break from the same old, same old, we opted to hire a babysitter instead of taking our kids along.

Back To School Supplies That Are A Waste Of Money (And What To Buy Instead)

Back to school season is here and it won’t be long before backpacks and homework folders are filling your entryway. Before you can fall back into your school year routine, you’ve got to do your back to school shopping. For many, that’s a huge expense.
Although the cost of school supplies has dropped a bit, most parents can expect to spend the same amount on preparing a single kid for their first day of school as they would on a single mortgage payment, according to the 2018 Backpack Index by Huntington Bank.
For kids in elementary school, the average cost of school supplies is $637. For middle school students, stocking up on binders, pencil, and paper is probably going to cost around $941, and for high schoolers $1,355. Kind of wild, right? Now imagine you’ve got not one, but three or four kids to send back to school fully stocked with clothing and supplies. For many, it simply isn’t realistic. It costs too much!

So, should you just bite the bullet, swipe your credit card, and worry about the bill later? Maybe not. Shopping for school clothing is by far the biggest expense parents face as they get their kids ready for the new school year. One report, compiled by the National Retail Federation, estimates that parents are spending an average of $236.90 on school clothes per child.
One example of overspending is buying school uniforms through the school sponsored retailer, at a specialty uniform store, or even a high-end retailer. These high-cost uniforms quickly add up when you’re trying to purchase enough items for a full week of school. Take Land’s End, for instance, a popular retailer of school uniforms. Polo shirts cost as much as $35.50 each and khakis as much as $45.50.
The good news is that this expense can be lessened by buying the right things from the right retailers. These uniform options still meet most school guidelines and won’t totally deplete your back-to-school budget.

One of the most surprising expenses parents face at the start of a new school year is the cost of stocking up on the little, inexpensive supplies. A few dollars here and there for crayons, pencils, and glue might not seem like a big deal in the aisle, but you’ll watch your total quickly add up in the checkout line thanks to big differences in price between name-brand and store-brand options.
Even more shocking, perhaps, is that fact that Amazon isn’t the best place to stock up on the little stuff. Most lower-grade students are expected to show up to school with crayons. Although this school supply isn’t typically considered a huge expense, it’s a good idea to save money whenever possible! We don’t have to tell you how quickly the cost of a smartphone can add up. A new iPhone can set you back up to a $1,000 and comes with monthly expenses to pay for minutes and data usage. There is also a good argument to be made that giving kids a smartphone to take to school isn’t a great idea, even if they’re in middle school. Wait Until 8th is an organization that advocates for delaying giving kids smartphones for as long as possible, ideally until they’ve reached eighth grade.

On Parenting In A World Of Distraction

Recently, my 2-year-old son has started this new habit. He walks up to where I’m sitting, and he pulls on my arm. Specifically, he pulls on the wrist that is attached to the hand that is attached to my smartphone, and he asks me to get up.
If I’m being perfectly honest, it frustrates me. I don’t feel like I’m constantly on my phone, and typically I am trying to accomplish something for work when he interrupts. It wasn’t until more recently that I even considered that my distraction might be the problem—I kind of assumed he was just being a 2-year-old.

I’ve worked from home for four years now. Even though I do have childcare pretty regularly, I also do a lot of multitasking while my kids play or sleep. I’ve worked really hard to try to maintain some kind of balance, to draw lines between work and home, but that hasn’t always gone well.
It wasn’t until my son began this habit of pulling on my arm that I realized this might be one of those seasons that isn’t as in balance as it should be. It wasn’t until I noticed how much frustration I felt being pulled away from my email or texts that I became open to the idea that I might not be doing the best job keeping the lines between motherhood and work in check, and that I might be letting tech become an unhealthy distraction.

Redefining Balance in a Digital World

When I first started to research tech distraction and parenting, I was nervous. My expectation was that I was going to find a mountain of evidence that I needed to draw hard lines between work and home. I expected I was going to walk away from my research believing I only had one choice—to give up stay-at-home parenting altogether and hire full-time care so I wasn’t so distracted by my tech devices around my kids.

This wasn’t what I found to be true. Instead, I found experts are calling not for a complete absence of technology in family life, but a smarter, healthier approach to using and talking about technology. They’re calling for more intentional integration.

The Problem with Tech Distraction

Before I can get serious about changing the way our family approaches tech, I know I need to get clear on what problems tech-distraction creates for the modern family. Knowledge is power, right? In this case, the hope is that information will motivate lasting change in our family.

There is a lot of research on screen time and how it impacts children, but the body of research on parental use of screens and how it impacts family is still developing. What I find interesting about the research that does exist is its logical progression.
First, there is research confirming that children are more likely to act out when their parents are distracted by tech. Specifically, a study published in Child Development reported that when parent-child interactions are frequently interrupted by technology, parents are more likely to report that their children have behavioral problems.

Another study published in the journal Pediatrics observed families in a fast food restaurant, making note of their smartphone use and how it appeared to affect their interactions with their children. Researchers noted that 40 of the 55 observed parents used their smartphones during their meal. When left to entertain themselves (because of parental engrossment in technology), children were more likely to seek out their parents’ attention.

“Parents who are distracted by their smartphones are more likely to see kids misbehaving because kids will have to misbehave in order to get their attention,” explains Shane Gregory Owens, PhD, a board-certified psychologist who works with families and young adults. “Parents with kids who learn that the fastest route to attention is misbehavior will end up with kids who misbehave.”
In this same study, we see another consequence of tech-distraction—parents who appeared to be very distracted by their phones were more likely to respond harshly when their children acted out or tried to get their attention.

Most impactful, perhaps, is research on living in a digital world sponsored by AVG Antivirus. In 2015, they surveyed parents and children on family tech use and found that over half the children felt their parents were on a screen too often. Thirty-two percent of these children said the amount of time their parents spend on their phones makes them feel they are not important to their family.

Dealing with Our Tech Fascination

It is worth noting that many parents, like myself, are not unaware of the problem of technology in our lives. In the study mentioned above, 52 percent of parents admitted they were on their phones too often.
“My biggest concern … is that they’re really modeling behaviors that they’re not actually interested in their children picking up,” says Colleen Carroll, doctor of education and self-proclaimed screen freedom warrior. “It is not helping them as a parent to have credibility, and they lose the respect of their children.”

In my own life, and what I suspect is the case in many families, using tech less is easier said than done. I am drawn to social media when I am bored, lonely, or simply exhausted. Being on a screen provides me with a positive experience that keeps me returning again and again, even when I’ve committed to using tech less.
For this reason, dealing with our tech fascination just might be the first step to a more balanced approach to technology and family life. If you find yourself compulsively reaching for your phone, Carroll offers some practical steps you can take to make phones less interesting.
“Almost all smartphones have the option to make the screens black and white instead of color,” she says. “Color saturation is one of the main addictive qualities of devices today.”

Turning your phone to grayscale could be your first step toward becoming less fascinated with your phone. For those interested in making this switch, it’s an option usually found in the accessibility section of the general settings menu.
Next, permanently turn off all notifications on the apps on your phone. That way, you have to go out of your way to check emails, social media, or gaming apps.

Lastly, she suggests protecting periods of time devoted to other things, like work or time with family, by turning your phone on airplane mode. It’s a simple way we can all disconnect for a bit for the good of our mental health and our relationships.

Changing the Family Culture

The goal of a healthy relationship with technology is not to give it up completely. Disconnecting entirely isn’t just impractical, it’s impossible for most.
“The idea that any of us is going to be able to be very strict about work versus family time, at least right now, it doesn’t seem possible,” says Owens.


Instead, Owens encourages parents to be very intentional about how they use tech at home. Parents who hope to see a positive change in the family dynamic surrounding technology need to step back and take a look at the family culture as a whole, according to Carroll. This can be accomplished by clearly defining your family values and then making concrete changes to center schedules and routines around them.

“Families have success by creating designated work times where everybody can be on devices at some point,” she says. “For some families, it is between 7 and 9 p.m.; for other families, it’s more like 4 in the afternoon and 6:30.”
This time can be used in a way that works best for the family. This is the time when parents can catch up on work, kids can complete computer-based homework, and families can enjoy entertainment technology together.

“Then there is an agreed upon shut down time so that everybody is also following that, and then they can do the other helpful activities that are really necessary on a nightly basis,” Carroll says, suggesting reading, bedtime routines, or even watching a show together as a family.
Building this schedule or routine is the perfect time for a formal discussion as a family about what the family values most and how they plan to make that a priority over screen time.

“The family should get together and say, ‘Here’s why we don’t spend five or six hours a night on screens or playing video games,’” she says, providing outdoor time, reading, or socializing together as activities that could be scheduled in place of mindless screen time.
These two suggestions are part of a larger point that both Owens and Carroll stress as important. Parents need to explicitly talk about technology with their children. Children will make assumptions about their family values based on what they see and hear from parents, and if we don’t talk about why we’re not devoting our lives to screens, kids will likely get caught up in being connected.

“The most important tool that any parent has is modeling the behavior they want to see in their kids,” says Owens. “So parents want kids to pay attention to them when they’re talking, so if your kids talks to you and you are on your phone, you put down your phone and you look at your child and pay attention to them until they’re done talking.”

What does this mean for me and my family dynamic? That’s a question I’m still exploring, beginning with talking with my spouse about how we want to start scheduling in screen-free family time on a daily basis. Meningitis is a serious condition that requires immediate attention from a medical professional. It can be life-threatening if left untreated, so it is important to understand the telltale symptoms. When you can spot symptoms early on, you can quickly seek out medical attention that can mitigate the negative effects of the disease.

Understanding the Types of Meningitis

There are a few different types of meningitis, but bacterial and viral meningitis are the two most common.
Bacterial meningitis is the most severe form of meningitis and can be fatal, especially if treatment is delayed. There are many types of bacteria that can cause meningitis, including Streptococcus pneumoniae, Group B Streptococcus, and Listeria monocytogenes.

Thankfully, the introduction of and increased access to safe and effective vaccines resulted in a steady decrease in bacterial meningitis cases since the 1990s. However, cases that do occur are dangerous and can be fatal if left untreated.
Bacterial meningitis is treated with oral or IV antibiotics, and treatment can last between 10 and 21 days, according to Allan Greissman, MD, of Pediatric Critical Care of South Florida.
The second most commonly experienced meningitis is viral meningitis. Although there is no vaccine for viral meningitis, you can be vaccinated against some of the viruses that could cause meningitis, like measles, mumps, or influenza.

It helps to think of viral meningitis as a potential complication of these other viruses. This means that, although you might catch measles, mumps, or the flu from someone with viral meningitis, that doesn’t necessarily mean you will also develop viral meningitis.
“Viral meningitis will run its course and should not [be], and is not, treated with IV antibiotics,” says Greissman. He notes that one exception is a form of viral meningitis caused by the herpes viruses, which is treated with an antiviral medication.
Other types of meningitis do exist—fungal, parasitic, and non-infectious—but these are rare when compared to the other two types.

The most important thing to understand about the different types of meningitis is that only a spinal fluid culture can provide an accurate diagnosis.
“I really would encourage people not to get too focused in terms of trying to differentiate [between types of meningitis],” says Christina Johns, MD, Fellow of the American Academy of Pediatrics and senior medical advisor at PM Pediatrics. “While sometimes they can present differently in terms of time or severity, the symptoms or the typical [experiences] are the same.”

Meningitis Risk Factors

Certain populations are more at risk for developing meningitis than others. Being unvaccinated for meningitis puts you at a higher risk for developing a meningococcal disease, according to Johns. Additionally, being immunosuppressed because of another condition, like HIV, increases the risk of developing infections in general.

Age is also a risk factor, with young children, young adults, and older adults at an increased risk for developing the disease.
Living in a communal setting, like a boarding school or military base, is another risk factor for developing meningitis. For instance, thanks to the close living quarters of college dorms, there have been recent infectious disease outbreaks on college campuses, as reported by the Centers for Disease Control and Prevention.

Symptoms Everyone Should Know

It may seem that meningitis presents much like less serious or more common illnesses, but there are a few symptoms that set this disease apart.

At times, the symptoms can seem subtle, according to Greissman, but they can rapidly escalate, especially in the case of bacterial meningitis. This is why he recommends that any patient presenting with the following symptoms seek immediate medical attention, especially if the patient is an infant who cannot verbally share what they are experiencing.

Stiff Neck

Because this infection causes inflammation, neck pain (and general muscle soreness) is one of the primary symptoms of meningitis, according to Greissman. Although it might be true that other viral and bacterial illnesses cause neck pain, the neck pain associated with meningitis is on a distinct level.

“It’s not just, ‘oh my neck is sore,’ but really, I have likened this clinical picture with people kind of looking like they’re as stiff as a board and moving as a unit,” says Johns. “It really hurts to turn their neck in any direction.”
The same inflammation that causes neck and muscle pain also causes meningitis patients to experience extraordinarily intense headaches. These headaches will steadily become more intense and medication often won’t provide notable relief.

Sensitivity to Light

One of the earliest warning signs of meningitis is photophobia. An individual may develop a dislike of or sensitivity to all light, but especially brighter lights. In addition to eye discomfort because of light, individuals experiencing photophobia may develop headaches and nausea.

Since photophobia is an early symptom of meningitis, it is certainly worth taking seriously as a cue for seeking medical advice, especially when experienced alongside other symptoms.

Sudden High Fever

A high fever is one symptom of meningitis that is admittedly subtle or easily mistaken for another illness. Even so, it is an important symptom to take seriously, especially in young children. For children too young to communicate that they have a sore neck, are experiencing photosensitivity, or have extreme headaches, a fever is one of the most objective ways to determine if that child needs to see a doctor.

Kathy Fray, former midwife, parenting author, and private maternity consultant, says that high fevers of 100 degrees or more in children under six months always warrant medical attention. In children over the age of six months, a fever that persists after a dose of acetaminophen or continues to escalate warrants a medical assessment.

A Persistent Rash

The presence of a distinctive rash is a symptom of a more developed case of meningitis and indicates that the infection has progressed to a dangerous point. The rash that develops is caused by broken blood vessels under the skin, according to Fray, and it is an indication that the individual is now experiencing septicemia, otherwise known as blood poisoning.

When the infection has gotten to this point, the rash will appear all over the skin. While many other viruses and infections might come with a rash, the rash associated with meningitis quickly appears and spreads. What differentiates this rash from others is that it is under the skin, not on the surface. The rash may even begin to look more like bruises than hives, according to Healthline.
An at-home test using a clear drinking glass can help determine if the rash is on the surface or under the skin. When you push the glass against a typical rash, it actually blanches (meaning the color disappears), explains Fray. In the case of a rash associated with meningitis, it doesn’t blanch, since it is under the skin.

The rash may be harder to spot on darker skin tones. To be certain, check paler areas of the body, like the stomach, the palms of hands, or the soles of feet.

Changes in Mental Status

As meningitis escalates, some patients experience changes in mental status or personality. They may become confused or extremely lethargic, according to Johns.
“A change in mental status in this situation can usually be described as a decreased level of responsiveness; true lethargy or difficult to arouse,” she says. “It’s much more than ‘sleepier than usual.’”

It is also fairly common for meningitis to cause unusual irritability, according to Greissman. Because of the common occurrence of irritability and agitation in young children with meningitis, when this symptom is experience in combination with a high fever it warrants evaluation for meningitis, even when the child does not or cannot report a stiff neck, according to research published in the European Journal of Pediatrics.

Seizures

Some people who contract meningitis will develop seizures that typically subside quickly after treatment. Seizures associated with meningitis mimic seizures with other causes: The person may stiffen up, experience muscle spasms, or even lose consciousness.

What sets a seizure apart in meningitis is the cause, according to Johns. For instance, with epilepsy, seizures are caused by an abnormal electrical impulse within the brain tissues. In meningitis, however, seizures result from the brain being irritated from the infection.
If your child or loved one is experiencing a seizure, especially in the presence of other symptoms, seek medical help right away. Until help arrives, roll the person onto their side, place something soft under their head, and stay with them. Do not restrain them or put anything in their mouth, as this could lead to choking.

Rapidly Escalating Symptoms

One final thing that sets meningitis, especially bacterial meningitis, apart from other less-serious illnesses is that it escalates rapidly, according to Greissman. The symptoms above may be confused for another illness at first, but they become dramatically worse over a short period of time.

Meningitis Prevention

The most effective form of treatment for bacterial meningitis is following early childhood vaccine protocols.

“In the year 2018, given all the vaccines that we now give, bacterial [meningitis] is much less common than 20 years ago,” says Greissman. “We now have vaccines for the pneumococcal bacteria, the meningococcal bacteria, and haemophilus influenzae bacteria.”
When it comes to viral meningitis, there aren’t specific prevention protocols since there is no vaccine. Instead, Johns reminds us that preventing the spread of viruses comes back to the basic practices that prevent the spread of any disease. Wash your hands well, avoid touching your face, and stay home from work or school if you don’t feel well.

Can collagen-supporting products keep you looking younger and improve your overall health? Or are they simply a chance to make money off impressionable health-minded consumers?

Understanding Collagen

Collagen is a protein that has many jobs in the human body. For starters, collagen serves as connective tissue for bones, skin, muscles, and more. It also plays a role in the blood clotting process and keeps skin looking young by giving it elasticity.

It’s the most plentiful protein found in animals and humans, with at least 16 existing types of it, according to research published in Molecular Cell Biology. Types I, II, and III, though, account for between 80 and 90 percent of the collagen present in our bodies.
One of collagen’s primary responsibilities is forming support systems for the body. Without it, muscles, tendons, bones, skin, organs, and blood vessels simply wouldn’t have the elasticity or strength they need to do their jobs.

Along with chondrocytes, proteoglycan, and elastin, collagen is one of the four substances that make up the cartilage in your joints. Cartilage, of course, acts as a padding between bones, reducing friction between them and absorbing shock—collagen keeps that cartilage strong.
Our bodies naturally produce collagen from the nutrients we consume. The production process, like that of most proteins, requires ample amino acids, particularly glycine and proline. In order to turn these amino acids into collagen, our bodies also need vitamin C.

If you’re eating a well-balanced diet, the body will have everything it needs to make collagen, according to board-certified dermatologist Janet Prystowsky, MD, PhD. But as our bodies age, it’s normal for collagen production to slow down. The collagen we do have often breaks down during the aging process, as well. Fear of this process leads some people to turn to collagen supplementation—more on that later.

Signs Your Collagen Production Has Slowed Down

Aging bodies produce less collagen and might exhibit one, or all, of these telltale signs or symptoms that collagen breakdown is taking place.
When the breakdown of collagen occurs, skin elasticity decreases. This causes the skin to sag, according to Prystowsky, and when you press on it, it won’t bounce back. It also leads to some skin wrinkling with age.

When you’re experiencing collagen breakdown, your joints can also suffer, according to Luiza Petre, MD, a board-certified cardiologist and weight management specialist. “You will experience lack of joint stability, joint stiffness, and pain,” she says, adding that achy and sore muscles also come with the territory.

Petre says collagen breakdown and slowed collagen production are also associated with thinning of hair and gum recession. One 2009 study even linked collagen degradation with the inflammation that occurs from periodontal disease. It is important to note that things like gum recession and thinning hair aren’t exclusive to collagen breakdown, but collagen breakdown can play a role in these processes.
While some collagen breakdown is normal, it is important to help your body create the collagen it needs to keep you strong and healthy. Aside from supplements and powders, there are some foods you can eat to foster your body’s natural collagen production.

Supporting Collagen Production

The body needs certain nutrients to keep up with collagen synthesis, and whole foods are the best way to give the body what it needs.

Amino acids are essential to the collagen production process. Because amino acids are the building blocks of proteins your body needs, foods high in protein will help you get plenty of these nutrients. No matter your tastes or dietary preferences, there’s a protein-rich food for you; you can get amino acids from meat and seafood as well as plant-based protein sources like legumes.

Cabbage, kale, cauliflower, and spinach are among the many vegetable sources of amino acids. Fruits like apricots, bananas, and goji berries also contain amino acids.
The body needs vitamin C to complete the collagen production process. Vitamin C is plentiful in brightly colored fruits and vegetables, including citrus fruits, berries, avocados, and leafy greens.

Additionally, zinc aids in collagen production, according to Petre. She recommends getting zinc from wild-caught salmon. Vegans and vegetarians can obtain zinc from legumes and oats or vegetables like spinach, mushrooms, and asparagus.
Petre is also a huge advocate of including greens in your diet for another collagen-boosting nutrient.

“Greens such as broccoli, bok choy, green beans, arugula, lettuce, green algae, and kale have chlorophyll, which gives plants their color,” she explains. “Studies show that consuming chlorophyll increases the procollagen.”

The Great Collagen Debate

Once we start talking about collagen supplementation and nutrition for collagen production, things get a little controversial. In our research, we learned there is much debate about the benefits of ingesting collagen supplements.
“Your body needs collagen, but you actually make it on your own, so most of us don’t actually need to be adding collagen powder to your foods or taking it as a supplement,” Nicole Osinga, a registered dietitian, told Reader’s Digest.
Prystowsky agrees. She says collagen supplementation doesn’t make sense given what we know about the body. She explains that ingesting collagen doesn’t really translate to more collagen in the body because ingested collagen will be broken down into individual amino acids during digestion. Those amino acids will be absorbed and used by the body for various processes, including the production of collagen.

On the flip side, Petre advocates for ingestion of bioavailable versions of collagen, or collagen that is most quickly used by the body.
“The best [diet addition that gives] collagen supplies a boost is bone broth,” she says. “It contains a form of collagen that is bioavailable that your body uses immediately.”

Our experts’ differences in opinion are fairly representative of the opinions that exist about collagen supplementation in the medical and nutrition world. Many believe there are no benefits; others see the potential. Research is still ongoing.
On the pro-supplementation side of the debate: One 2014 study published in the journal Skin Pharmacology and Physiology found evidence that collagen peptides supplementation is associated with greater skin elasticity. And in a 2015 study published in The British Journal of Nutrition, researchers reported improved muscle strength in men who took collagen peptides during strength training compared to those who took a placebo.

The good news is that there isn’t any evidence to suggest collagen supplements are bad for you—yet. Unfortunately, most research is in the early stages. Studies have been small in scope or short in duration. More research is needed before we can say with certainty that collagen supplementation helps.
However, the supplements probably aren’t dangerous. While they may cause mild side effects, as any supplement has the potential to do, and they aren’t subject to FDA review, they are more than likely safe. As for their level of overall usefulness, well, that’s still up for debate.

Living a Collagen-Friendly Lifestyle

No matter what side of the collagen debate you find yourself on, it’s pertinent to point out that there are proven ways to support the collagen already in your body. How To Read Your Baby (And Gain Their Trust)

For parents of a new baby, deciphering what they need based on a series of subtle cues and seemingly identical cries can be overwhelming. As a new mom, I found a lot of comfort in developing a pattern of feed, sleep, and play, with diaper changes sprinkled in the mix, but there were still plenty of instances when I found myself confused to the point of tears.

It often felt like nothing I tried would calm my baby or help her drift to sleep. With time, I learned that my baby was actually an excellent communicator—I simply needed to learn to watch for patterns in how she expressed her needs. My daughter wasn’t extraordinary; babies actually begin communicating with their parents from a very young age. Want to learn how to read your baby? Here’s what you need to know.

The Development of Communication

The foundation of communication is being built long before babies are born, according to Ayelet Marinovich, pediatric speech-language pathologist and parent educator.
“We know that even babies in utero are starting to use their sense of hearing,” she says. “So they’re listening to things like the components of speech and the melody of the language or languages.”

This learning begins around 20 weeks into the pregnancy.
From birth, developing children tend to hit specific communication milestones, both verbal and nonverbal. One of those first milestones, seen roughly around 6 weeks post-birth, is a social smile. The baby observes someone smiling at them and returns the smile, noting that their smile evokes a positive response from the recipient.
“They can recognize … ‘Oh! That gets a response,’” Marinovich says, noting that crying functions very similarly, with babies learning quickly what their cries achieve.

At 4 to 6 months, babies coo, indicating that they are learning vowel sounds. This is followed by babbling, which is all about learning to use vowels and consonant combinations. At 9 months, babies begin using gestures to draw attention to things.
And around 12 months, the first words are spoken.

Listening and Watching for Subtle Cues

A year is a long time to parent a baby without the assistance of words. The good news is that words aren’t the only form of communication, and babies use crying, body language, and facial expression to give voice to what they want and need most from their caregivers.

Although many babies cry if they’re hungry, most babies start dropping hints long before things feel urgent enough for tears. Cries are actually not considered hunger cues; instead, they signal distress, according to WIC Works (link opens a PDF), a project of the United States Department of Agriculture. According to this resource, parents can avoid distressing hunger in their children by learning patterns and subtle cues babies use to draw attention to their hunger.

In newborn babies, rooting around for the breast, sucking on their hands or fingers, and drawing their hands to their face are all signals of hunger. Most babies also open and close their mouth when they’re ready for a meal.

Typically, babies are ready for solid foods once they are sitting up on their own. Parents can gauge their baby’s interest in solid foods by watching for common signs like reaching for what is on their parents’ plate or the ability to handle foods without pushing them out of their mouths.

I’m full!

Just like babies signal their parents when they are hungry, they also communicate when they are satisfied. This is an interesting and important topic, as properly reading infant cues on hunger and satiety is considered part of obesity prevention. It helps avoid over-consumption, according to a research review in the journal Maternal & Child Nutrition.

Newborns typically slow their sucking or even fall asleep when they are satisfied, according to WIC Works. They may turn their head away and appear more relaxed. As babies grow older, they become distracted or want to play while eating. Some babies clamp their mouth shut to refuse a bottle or breast.

I’m tired!

If you’ve ever cared for an infant, you know there is such thing as an overtired baby. Some babies become so exhausted it is actually more difficult to get them to sleep. Watching for cues that your baby is ready for a nap or bedtime can make the whole experience less dramatic for everyone involved—saving you from a bedtime struggle or hysterical crying from a wiped out baby.

Most obvious, perhaps, is that newborns yawn when tired. They also pull their hands to their face, clench their hands into fists, rub their eyes, or pull at their ears, according to Today. As babies grow more tired, their movements become jerkier and they become less interested in their surroundings.
Catch sleepiness early, use a routine to make naps simple and predictable, and encourage healthy sleep habits in developing babies.

I need quiet.

From a young age, babies drop hints when their environment has become too much for them. Like older children and adults, babies need quiet and breaks from stimulation.
“Maybe they’re closing their eyes,” says Marinovich. “Or they’re crying for ‘no apparent reason.’”

Many babies will also look away, wave their hands, or kick their feet if they’re becoming overstimulated. The solution to overstimulation is as simple as making an adjustment to their environment like lowering or removing noise, stepping away from them for a time, or lowering the lights in the room.

One of the primary jobs that comes with being a baby is learning by interacting with their brand new environment, according to Marinovich.
“The baby is in this sensory world, they’re all of a sudden exposed to noise and everything they’re seeing and everything they’re touching,” she says. “This is a very different environment than what they had in the womb.”

As they watch, listen, and learn, babies are focused on finding patterns in their environment and indications of what deserves their attention, Marinovich explains. Having a parent present and close gives them an opportunity to learn through interaction.

Of course, newborns don’t exactly toss a ball or drag out a train set when they’re looking for a little one-on-one with their caregiver. How can you know when they’re up for some interaction? According to Marinovich, babies indicate this even within their first week of life by settling into an awake, but calm, state of being.
“Babies sort of have these patterns of alertness and quiet,” she says.

I’m in pain.

Babies do so much crying, it is difficult to determine what their cries mean exactly. Are they tired and hungry, or is something causing them pain? Crying that indicates pain escalates in intensity and doesn’t resolve with comfort or food. A cry caused by pain may also have a higher pitch than usual, according to Kyla Boyse, RN, of the University of Michigan.

Parents can also watch for body language typically associated with pain. Infants in pain scrunch up their faces, and their body may become tense. Determining the source of pain is difficult, but a common cause is gas or an upset stomach. If you are concerned about your baby, a doctor can help determine what is causing the pain and if further treatment is necessary.

Encouraging Strong Lines of Communication

A strong bond between an infant and their caregiver is foundational to their emotional and physical health. One popular school of thought on child attachment, developed by German psychologist Erik Erikson, argues that the first lesson children must learn is that the world and the people around them are trustworthy. For this reason, understanding how to read your baby and responding to their needs is an important part of building a healthy relationship with your child.

The good news is that encouraging strong lines of communication with an infant or young child isn’t rocket science—it’s really about being present and consistently tuned in to their needs. Marinovich strongly believes that parents are already equipped with the tools they need to bond with their baby, and she says many parents are already doing many of the things necessary to encourage communication.

“We tend to overcomplicate things,” admits Marinovich. “We can see how they are communicating by these early cries … we can offer comfort through things like our touch and smell and voice and milk.”

One specific example is the use of infant-directed speech, which is often referred to as motherese. Listen to yourself talk to a baby for even a few seconds, and you’ll probably hear it—you may slow down your speech, speak in a higher pitch, or add a kind of sing-songy component to your speech. This habit is universal, according to Marinovich, and doing so encourages more attention from infants.

There is also research published in the journal Infancy that suggests infant-directed speech contributes to language acquisitions and is specifically helpful for learning the skill of word segmentation.
Lastly, consistent communication with your newborn can easily be included in your routine. Think about the things you do day in and day out with your baby and how talking and play could be introduced into those tasks.

The Origin Of The Apgar Test And Newborn Health

As an expecting mother, there is nothing you want more than a healthy baby. When the time finally comes for you to greet your baby for the first time, there is nothing you hope for more than hearing those first few cries and then hearing the doctor confirm that all is well. And when the time comes to take your child home from the hospital, you want to leave with the confidence that your baby will thrive.
That’s where the Apgar test comes in. Using a one to 10 scale, the standardized test is meant to measure how well the baby is doing after birth.

Most moms who have given birth in a modern hospital are familiar with Apgar scores. Some mothers have smiled in joy hearing that their newborn was scored at an eight or a nine, while other mothers have furrowed their brows in concern when the doctor says the baby has scored a lower number.
Here’s what you need to know about the test, its origins, and what your baby’s score means for their health.

What are the origins of the Apgar test?

The Apgar test was named after its creator, Virginia Apgar, MD. After spending many years working as a surgeon, Apgar experienced a career shift. In 1949, she was named a professor of anesthesiology at Columbia University’s College of Physicians and Surgeons, according to Profiles in Science published by the U.S. National Library of Medicine.

Her new work provided the opportunity for research, and it piqued her interest in maternal anesthesiology. More specifically, she was interested in spending more time researching mortality rates in newborn babies.

By 1952, she had developed a scoring system used on newborn babies during the first minute after their birth. The main goal of her system was to see what effects labor, delivery, and maternal anesthesia had on the well-being of brand new babies. Her attendance of over 17,000 births informed her development of the test.

Today, the Apgar test is standard in modern hospitals. Apgar’s system for scoring newborn health within the first five minutes of life is used all over the world.

What does the Apgar test measure?

Although the test was originally named after its creator, it was later developed into an acronym describing what it specifically measures. Each letter of the test’s name indicates one aspect of newborn health that is given a score between zero and two: appearance, pulse, grimace response, activity, and respiration.

During the first minute after birth, the evaluating nurse, doctor, or midwife scores the baby’s appearance first by looking at their skin tone. The baby will receive a score of zero, one, or two. A score of zero indicates a blueish pale color, while a score of two indicates pink skin all the way to fingers and toes, according to the U.S. National Library of Medicine.
Next, the medical professional checks the newborn’s pulse. Babies with a heart rate over 100 beats per minute, a healthy heart rate for a newborn, receive a score of two, and babies born with no pulse receive a zero.

The grimace response measures the baby’s reflexes, or how they respond to stimulation like a mild pinch, according to the U.S. National Library of Medicine. Babies who pull away have strong reflexes and are scored with a two, while a one is given to babies who simply grimace, and a zero is given to babies who don’t respond at all.

Activity refers to a baby’s muscle tone. Babies who appear floppy or aren’t moving are scored a zero, while babies who are moving often are scored a two.
Lastly, respiration is measured based on the baby’s breathing rate and the strength of their cry. A strong cry earns a score of two, while a baby who doesn’t cry at all will be scored at zero. An audible but weak cry is given a score of one.

These individual scores are added up to create one score given to the baby. Babies who score seven or higher on the Apgar test are considered healthy, while a lower score may indicate a problem or the need for immediate medical care.

What are the limitations of the Apgar test?

“The Apgar test helps determine how well a baby is immediately after birth and if a baby requires resuscitation or support breathing,” Yvonne Bohn, MD, OB-GYN at Providence Saint John’s Health Center in Santa Monica, California, says.
However, babies do undergo additional testing and monitoring before they leave the hospital with their parents, usually within the first day or two of life. Healthy babies without exceptional medical needs will all undergo the same three tests at some point during their hospital stay, according to Baby’s First Test—a blood test, a hearing test, and a pulse-oximetry test. Additional testing is only performed if the parents request it or the doctors have concerns about the baby’s health.

The standard newborn hearing test is actually two different screenings that take between five and ten minutes to complete. One tests the hearing nerves and how they respond to sound. The other actually uses a probe placed in the baby’s inner ear to measure sound waves.
The blood test, also known as the heel prick test, is meant to monitor for genetic conditions long before symptoms might be noticed, according to Erin O’Toole, a certified genetic counselor who works with expecting parents and parents of newborns.

“Conditions on the newborn screen all have a much better prognosis if treatment is started before symptoms are present,” she says. “Each state runs their own newborn screen, so depending on where you live, the baby is tested for different conditions.”
The pulse oximetry test is completely non-invasive. A nurse will place a monitor on the baby’s foot or hand that measures both their heart rate and the oxygen in their blood. This test is used to catch early symptoms of congenital heart defects.

The Apgar test is generally seen as a reliable first look at the baby’s health. Labor and delivery can be a stressful experience for babies, especially in high-risk pregnancies, difficult labors, and cesarean sections. A lower Apgar score is more common in these cases.  Babies born prematurely are also more likely to have a lower Apgar score.

Even though it has become the standard, it is not a perfect test. One of the major limitations is that not all measurements are exact. Sure, the respiration rate is a hard number, along with the pulse, but the rest of the factors measured are fairly subjective.

“These measurements are subjective and the score may be different based on the observer,” says Bohn. So, one doctor may see a great reflex response while another sees a response they believe is not as strong.
At this time, Bohn says there are no new newborn tests being developed, but O’Toole does say that the newborn blood tests are becoming less expensive and more accessible. She hopes this will result in better outcomes for children with genetic disorders over time.

What do I need to know about my baby’s Apgar score?

Regardless of your baby’s score, they will be tested again at five minutes.
“The hope is that the score will be at least seven at the five-minute mark to help the medical team determine how the baby is transitioning to breathing outside the womb and decide when to intervene,” explains O’Toole.

After the test at five minutes, babies with a score of seven or less will continue being tested.
During this time, some babies might require additional support to aid breathing. A member of your baby’s care team may also take a gas sample from the umbilical cord, according to O’Toole. From this point, medical treatment will continue if the baby’s score doesn’t climb to seven or higher.
For parents, it is important to remember that even a low Apgar score isn’t the final word on a baby’s health. Some babies score low immediately after birth because they are having a more difficult time transitioning into the world, according to O’Toole. It’s helpful to think of this test as an answer to the question, “What does this baby need right now?”

Remember, those first few minutes are important, but they’re not always an indication of what a baby’s next day, year, or entire life will be like. Should Children Be Playing Contact Sports? The Risks And Benefits Of Youth Athletics

“Do I love hockey more than I love my child?”
This is the question Bennet Omalu, a neurologist and forensic pathologist, asked in his book Truth Doesn’t Have a Side. Omalu is famous for publishing groundbreaking research documenting the damaging effects of football on professional athletes in America. His research changed the way we think about contact sports. In his opinion, no person under the age of 18 should participate in contact sports. The risk, in his mind, is simply too great.

In a country in love with sports, this claim has caused many parents to pause and ask hard questions about their decisions regarding their child’s participation in sports. Should all adolescents be withdrawn from contact sports completely? What is the real risk of continuing to let them play? And on the other hand, could there be risks associated with refusing to let your child play team sports?

The Risk of Joining the Team

There is no denying that there are risks associated with participating in contact sports. Recently, a Texas NBC affiliate published quotes from doctors warning against preventable injuries caused by sports. Their specific worries concerned overuse injuries (injuries caused by too much stress on a certain body part).
According to the article, these injuries are so common, they’ve reached “epidemic levels.” Doctors said these injuries could be prevented, however, by scaling back on training in childhood sports.

The risk of traumatic brain injury is also gaining national attention.
Participating in competitive sports puts young athletes at a high risk of sustaining at least one concussion in their lifetime, according to research published by JAMA: The Journal of the American Medical Association in 2017. Of the survey’s 13,088 adolescent responders, 14 percent reported sustaining at least one concussion. When responders also reported participating in competitive sports, that number increased.

This isn’t surprising, considering that sports accounted for roughly half of all concussions in adolescents, according to a 2013 report published in JAMA.
The good news is that concussion symptoms are typically temporary. It isn’t common for adolescents to experience lasting effects from a properly treated concussion, according to the Mayo Clinic.

The real concern about concussions pertains to the risks that come with repeated or compounding concussions. One study performed pre-season cognitive testing on high school adolescents, examining for symptoms of impairment and asking students to self-report concussions. The adolescents who reported more than one concussion were more likely to exhibit cognitive, sleep, and physical symptoms. The symptoms for students reporting three or more concussions were more likely to be significantly more severe.

The Risk of Benching Your Kids

All three experts who contributed to this article are in favor of participation in high school sports despite the risks involved. When safety precautions are taken, experts believe sports are an important part of the adolescent experience.

The experts also point out that non-participation is not without danger. Parents may believe they are keeping their kids safe from health problems, but there are real risks associated with benching kids, too.

“We’re living [at] a time when we have an obesity epidemic,” points out Mark McLaughlin, MD, who practices neurological surgery at Princeton Brain and Spine Care. “Keeping [children] out of sports for fear of risk of concussion is doing them a disservice.”
Just participating in one team sport in high school can significantly reduce students’ obesity risk, according to The New York Times. There is also convincing evidence that team sports participation is linked to success in school, capable of teaching skills on the field or court that translate well in the classroom, according to a University of Kansas study as reported by KU News Service.

Through participation in team sports, students learn discipline, persistence, and resilience in the face of adversity, says McLaughlin. Sports are also an opportunity to improve physical fitness through increased strength, balance, and endurance.
“In many cases, it seems that a lot of the narrative has been around what contact sports can do to an individual,” agrees Vernon Williams, MD, sports neurologist and director of the Center for Sports Neurology and Pain Medicine at Cedars-Sinai Kerlan-Jobe Institute in Los Angeles. “I think we need to widen that narrative and discuss the things that sports in general, including contact sports, can do for the individuals participating.”

According to Williams, if withheld from team sports for fear of possible injury, children would miss out on numerous benefits, including improved physical fitness, a lowered risk of obesity, and improved cognitive function. He also points out that being on a team teaches cooperation, how to overcome adversity, and many other skills important to being a productive member of society.

Pediatric neuropsychologist Katie Davis, PsyD, adds that sports are an important part of the school experience.
“I think team sports … increase kids’ attachment to school,” says Davis. “So if they’re playing team sports, they have a real reason to go to school. They enjoy school and feel more connected to the school community.”

Keeping Kids Active and Safe

Deciding whether to withdraw kids from sports or involve them is not an easy choice. For parents who want their child to remain involved in team sports, however, there are a few guidelines they can follow to keep kids as safe as possible during practices and games.
One of the first things to know is that some contact sports simply aren’t worth the risk; McLaughlin doesn’t believe there is any reason for adolescents to be involved in boxing or mixed martial arts. In these sports, hitting someone in the head is part of the strategy.

“The goal of boxing is to give somebody a concussion,” he says.
Outside these two sports, McLaughlin says contact sports are getting safer for children. He notes that, because we have greater awareness, it may seem like concussions are becoming more frequent, when we’re really just getting better at detecting injuries. Additionally, thanks to the research on brain injury in contact sports, rules at the high school level are changing to increase safety.

To guarantee that the team a child plays for is implementing safe practices, McLaughlin advocates for high parental involvement in sports. He says it is currently lacking, but it is the only way to be certain your child is safely participating in their sport of choice. Parents should go out of their way to educate themselves on safety practices that can decrease the risk of injury in their child’s sport.
In the unfortunate instance that a child is injured while playing contact sports, the right response is crucial. Specifically, parents should take action and take their child to a health professional any time the child experiences loss of consciousness or memory impairment after a hit in sports.

More common symptoms include tiredness, headache, and sound and light sensitivity, but loss of conscious and loss of memory are the biggest indicators that the child needs immediate medical care, according to Davis.
“If someone has any symptoms of concussion, they should be removed from play, and they shouldn’t be allowed to return to play until they’ve been cleared … by the appropriate healthcare professional,” says Williams.

According to Williams, experiencing a second concussion before the first has healed poses serious risks to all athletes. Additionally, he notes that emerging research suggests sub-concussive brain injuries can add up over time to create some of the same effects a full-blown concussion can create.
For this reason, he pushes for reduced blows to the head in general in contact sports. He encourages parents to discuss ideas for reducing contact during practices with coaches as well as teaching athletes proper techniques. He also encourages athletic officials to enforce the new rules that have been created to minimize risks.

Lastly, Davis, who is a strong supporter of participation in team sports, believes monitoring should be non-negotiable.
“One thing that I think schools really need to implement … is requiring baseline cognitive testing for anyone who is going to play a sport,” she says. “Contract with a neuropsychologist and have them do baseline paper-and-pencil testing.”

This would create a baseline for all participating athletes, says Davis. That way, if they were to experience an injury like a concussion, trainers and coaches would have a starting point to measure the impact of that injury. Without this initial test, post-injury testing is inaccurate.
Ultimately, it is a parent’s choice if their child will participate in team sports. If you have any concerns about your child’s unique circumstances, it is always a good idea to consult with a doctor before allowing a child to become involved in activities with a risk of physical injury. With their help and proper education, you can confidently make a decision you feel is best for your child.
